A woman is charged with manslaughter, accused of a fatal crash that happened in 2022.
The probable cause affidavit for Alexix Alvarenga says she had meth and alcohol in her system while she was driving north on Highway 88, about 5 miles north of Inola on October 15th, 2022.
It says the victim, 56 year old John Herman, from Fayetteville, was driving south.
It says Alvarenga crossed the center line and hit the victim’s vehicle head-on.
Records show she was charged with misdemeanor DUI in Rogers County in May of 2022, 5 months before the fatal crash.
For that DUI, she was sentenced in April of 2023 to an 18 month deferred.
